You'll catch Wolves and Manchester in the audience shots at the start/end of "Born Slippy", most obviously.

Wolves 53m35 (smallish Civic Hall with balcony)
Manchester 63m14s (I think I'm the guy in the bright blue shirt, bottom left)
Manchester 63m19s (it's an arena show with a 'bowl' at the top)

It might be they filmed multiple shows from the stage for personal use, and added that footage in, with the main footage of the band from the other shows. I don't recall anyone with cameras at either Wolves or Manchester, though Alexandra Palace did have a guy on stage with a video camera.

I was pretty floored to hear that little bit of stage banter where Karl goes "let's have a look at everybody!" in a '98 bootleg, even though the songs before and after were completely different. I guess they just liked that line and inserted it in.

Yes, although the Vorst Nationaal gig provides the base audio for the album, there are inserts and interludes taken from other recordings.
