The mp3 must be called 060513_96k.mp3 (that's what its called at underworldlive.com), and in the same folder as the cue file.

I haven't tested this so I don't know but it does say that the latest version has only been tested on Tiger, and not Panther. Try v2.5.2 or v2.2.2 if the latest one doesnt work for you. I'll try and test it today on my G4 and see what's up.
EDIT: Ok, i've done some detective work on the Mac and you're right. It crashes both v2.6 and v2.5.2 when I click "Split!" (I'm using MacOS 10.4.6). So I downloaded v2.2.2 but that came up with a message saying its for 10.3 only. So I did what most people would do, and got out the Hex Editor

and changed the 10.3 string to 10.4 and ya know what? It loaded, AND it split the files without error. Gotta go now, but I'll be back later and post more instructions, or this hacked version of v2.2.2 for people to use.
